At Heartland PACE we believe that it is better for the well-being of older adults with chronic care needs to be served in the community whenever possible. We are committed to delivering bold, creative solutions to help care for and support older adults in the manner we would want for ourselves and our own family. Heartland PACE helps older adults live a meaningful life at home with grace and dignity.

With centers in Indiana and Kentucky, Heartland PACE was established to serve older adults with complex physical or medical needs who prefer to live independently at home.  We believe in always doing what is best for those we serve.

Under PACE, participants must receive all needed health care, including primary care and specialist physician services (other than emergency services) from the PACE organization or an entity authorized by the PACE organization.  PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE services.  Emergency services are covered.
